There are five things that work against "Lag" right now :
Computer :
The better the Computer is , the better it will be on the Game Side.
Internet :
The better the Internet Connection is , the better it will be on the Internet Side.
Comparation : Lan > Fiber Optic > DSL >= Cable > Radio > Sattelite > ISDN > 56K > 28K.
Wired > Wireless most of times.
Fiber Optic is the best of all right now. Some users say that they have 5-15 Ping when playing with Fiber Optic Connection.
DSL => Cable because Cable is a little better than DSL when not overcrowded but when overcrowded with users the performance will get way worse and DSL will be Better.
So Cable = Good but not Reliable , DSL = Good and reliable.
NIC :
The better the NIC is , the better it will be on the Game/Internet Side.
Specially if it has FNA for Offloading things like Firewall / VOIP
Programs from the Computer Processor.
Killer NIC did a very good job on this , creating a Network Card that offloads CPU Processing , has FNA and tweaks the packets so your gameplay will be better. Reducing Ping / Spikes to almost nothing and increasing FPS by a very good ammount. It completely destroys the Spikes.
Projects : Killer NIC.
Gaming Network :
Creating a Local Interconnection for Gamers is a good thing to do. It will reduce Hops / Traffic Congestion and will make the Gaming Experience more enjoyable.
By removing the Traffic Congestion it will reduce the possible Spikes and make the gaming faster.
Internet Side
Projects : Game Rail.
Gaming Platform :
There is a project made by Chinese Guys that tweak the game packets making you have almost no lag when playing with foreign people. It really reduces Lag to 0 , but it uses all your connection to do so... Making the Upload Requirements to be very high ( Almost four times more ). Still it has some Spiking but it is more a problem of Network Card than because of their system.
Internet Side
Projects : GG-Game and other Asiatic Starcraft Gaming Platforms.
Conclusion : By making a Combination with at last three of these things you can reduce lag to almost nothing. My advice would be to use all of them if you can .
On the Hardware Side you have Internet Connection / NIC / Computer and on the Software Side you have GameRail and GG-Game. Both ways are very interesting aproaches to reduce lag.

As nice as it sounds, lag will never be zero. This is a physical limitation due to the speed of light, that as of 2007, and the foreseeable future, we can't get around. The absolute best you can hope for is (2/3)c*line miles, where c is the speed of light (multiplied by 2/3 to get the approx speed of light in fiber) and line miles is the physical amount of fiber between you and your destination (not a straight line in most cases.)
Today, this "best case scenario" is tempered by switches and routing that can add on average 5ms per hop and you can see how latency adds up quickly.
